Slugging and TEWL: The Science of Occlusive Moisture Traps

Slugging is the rare viral skincare trend with actual clinical evidence behind it. Petrolatum reduces transepidermal water loss by up to 99 percent — no other over-the-counter occlusive comes close. But the same seal that repairs a mechanically damaged barrier makes inflamed or acneic skin worse.

Slugging is the practice of applying a thin layer of petrolatum — Vaseline, Aquaphor, or a generic equivalent — over your evening skincare routine as the final step, and sleeping in it overnight. It became a viral skincare trend in 2022. Most viral skincare trends do not have clinical evidence behind them. This one does.

Petrolatum reduces transepidermal water loss by up to 99 percent in controlled laboratory measurements [1][2]. That is not a marketing number. It is the actual mechanism by which petrolatum has been used in wound care and dermatology for over 150 years, since 1872 [3]. No other over-the-counter occlusive comes close — mineral oil reduces TEWL by roughly 30 percent, dimethicone by 20 to 30 percent, and plant-derived occlusives like shea butter by around 40 percent [1].

This article walks through what slugging actually does to a compromised skin barrier, why petrolatum works better than the alternatives, when slugging is the right tool for barrier recovery, and the three specific scenarios where it makes skin worse instead of better.


What slugging does — the mechanism

Petrolatum is a mixture of long-chain aliphatic hydrocarbons. When you apply a thin film to your skin, it does two things simultaneously.

It forms a physical seal. The film is hydrophobic — water cannot pass through it. Every water molecule in the outer layer of your skin that would otherwise evaporate into the air runs into the petrolatum film and stops. The evaporation gradient that drives transepidermal water loss is blocked at the surface. Water stays where it is.

It penetrates the outer skin layer and modulates barrier repair. This is the part most people do not know. Petrolatum is not just a passive lubricant sitting on the surface. It diffuses into the spaces between the outer skin cells, where it upregulates the expression of specific barrier and antimicrobial genes [4]. In studies of skin with atopic dermatitis, petrolatum increased expression of the differentiation markers filaggrin and loricrin, thickened the outer skin layer, and reduced T-cell infiltrates — all measurable structural improvements to the barrier itself [4]. The seal is not the whole mechanism. The seal buys the time and conditions for the underlying repair to happen.

The two effects together are why slugging produces changes that outlast the overnight application. The immediate effect is water retention. The longer-term effect, over consecutive nights, is a barrier that has been given the space to rebuild itself under a stable seal.


Why petrolatum specifically

There are many occlusive ingredients in skincare. The reason petrolatum is the reference standard is that no other widely available ingredient comes close to its TEWL reduction on a head-to-head basis:

  • Petrolatum — reduces TEWL by up to 99 percent [1]
  • Lanolin — reduces TEWL by roughly 65 percent
  • Shea butter — reduces TEWL by roughly 40 percent
  • Mineral oil — reduces TEWL by roughly 30 percent
  • Dimethicone — reduces TEWL by 20 to 30 percent [1]

The reason for the gap is molecular. Petrolatum's long-chain hydrocarbons form a more complete and more stable film than any other cosmetic ingredient in this category. It stays on the skin longer, resists dissolving in sweat and sebum, and blocks water vapor almost completely rather than partially. It is not a marginally better occlusive. It is the occlusive that all other occlusives get compared to.

Dimethicone has real value for daytime and lighter applications — it wears cleanly under makeup and sunscreen, does not feel greasy, and reduces TEWL enough to make a difference on mildly compromised skin. But for a severely compromised barrier during active recovery, dimethicone is not delivering the same order of magnitude of water retention. Petrolatum is what closes the leak.


When slugging is the right tool

Slugging works best when the underlying barrier problem is mechanical damage with low inflammation [5]. The petrolatum seal traps water, blocks environmental irritants, and gives the skin the stable conditions it needs to repair the outer wall without ongoing insult.

The specific scenarios where the clinical evidence supports slugging:

Post-procedure recovery. After chemical peels, laser treatments, microneedling, or extractions, the outer skin is mechanically disrupted and the barrier is running at high TEWL. A 7 to 14 night course of nightly petrolatum during the peeling and re-epithelialization phase is the textbook intervention [5][6]. Inflammation is usually low during this window, sebum production is suppressed by the trauma, and the petrolatum seal supports normal healing.

Winter chapping and windburn. Cold, dry, windy environments produce high evaporation gradients that overwhelm even a healthy barrier. Nightly slugging during active weather-related damage restores TEWL to normal within days and allows the barrier to catch up.

Post-retinoid disruption. In the early peeling phase of a retinoid — tretinoin, adapalene, or over-the-counter retinol — the barrier is deliberately disrupted, and TEWL runs elevated for weeks. A short course of slugging on non-retinoid nights supports the barrier through the adjustment phase, and can significantly reduce the peeling-and-tightness cycle that makes many people abandon retinoids in the first month.

Eczema flare containment on non-flexural areas. For localized atopic dermatitis lesions on the hands, elbows, knees, or shins, petrolatum applied nightly reduces flare frequency significantly [7][8]. A 2022 randomized controlled trial found nightly petrolatum reduced flare frequency by 43 percent over 12 weeks in adults with moderate atopic dermatitis [8].

Severe xerosis in mature skin. In people over 65, sebum production declines and the outer skin thins, producing chronic dryness that does not respond well to standard moisturizers alone. High-occlusion therapy — including slugging — is a mainstay of dermatologic care for age-related xerosis.


When slugging is the wrong tool

The petrolatum seal that repairs a mechanically damaged barrier will make three specific scenarios worse. Understanding these prevents the second most common slugging problem, which is people slugging themselves into an acne flare and blaming the wrong ingredient.

Active inflammatory acne or sebum overproduction. When the underlying skin is producing excess sebum and has an active inflammatory acne component, the petrolatum seal traps the sebum and the bacterial flora against the skin. Bacterial populations under sustained occlusion shift toward acne-correlated species within 5 to 10 days [5]. The result is a wave of new breakouts, often around week two of a slugging routine, in someone who was slugging specifically to try to calm their skin. The petrolatum did not cause the acne — but it created conditions where existing acneic tendencies flared under the seal.

Fungal acne (Malassezia folliculitis). Slugging over skin with fungal folliculitis — the small, uniform bumps often clustered on the forehead, chest, or shoulders — provides ideal conditions for the yeast to proliferate. Petrolatum does not feed Malassezia directly, but the sealed, warm, humid microenvironment under the film is exactly what the yeast prefers. If you have suspected fungal acne, slugging is the wrong intervention. Antifungal treatment first, then reassessment.

Same-night retinoid layering. Applying petrolatum directly on top of an active retinoid dramatically increases the retinoid's penetration and skin contact time. This is not always a bad thing — some dermatologists use this deliberately for tretinoin patients who need more penetration — but it also increases irritation risk substantially. Layering petrolatum over retinoids without a physician's guidance can turn a mild peeling reaction into a moderate one, and a moderate one into a severe one. As a general rule, slug on nights you did not use a retinoid.

Perioral dermatitis, seborrheic dermatitis, and rosacea flares also generally contraindicate slugging — the sealed environment tends to worsen these inflammatory conditions rather than calm them.


How to slug — the actual protocol

Petrolatum works better when there is water underneath it to trap, and it works better on a base of physiological lipids than on bare skin.

Step one — cleanse cleanly. Whatever is under the petrolatum will stay there for eight hours. On days you wore sunscreen, makeup, or heavy pollutants, double-cleanse. On lighter days, one thorough cleanse is fine.

Step two — apply your usual PM routine. Serums, treatments, and moisturizer as normal. Petrolatum goes over everything, it does not replace anything. The most effective structure is a humectant serum on damp skin, followed by a ceramide-cholesterol-fatty acid cream (the same three-lipid formulation that supports barrier repair), then petrolatum on top.

Step three — apply a thin film. A pea-sized amount for the entire face is enough. Warm it between your fingertips first — cold petrolatum drags on the skin, warmed petrolatum spreads to a sheer film. Press it into the skin rather than rubbing.

Step four — sleep. The occlusive effect works passively overnight. TEWL normalizes within hours of application on a healthy site, and continues normalizing across the sleep cycle on a compromised one [1][6].

Step five — cleanse it off in the morning. Petrolatum does not fully absorb — some of it comes off on your pillow, and the rest needs to be cleansed away with your morning routine.


Dosing — how long, how often

Slugging is a precise recovery intervention, not a permanent nightly ritual.

For post-procedure recovery, retinoid adjustment, or winter chapping — a 7 to 14 night course of nightly slugging is the standard length, tapering to two or three nights per week once the acute barrier disruption has resolved [2].

For active eczema flares on non-flexural areas — nightly slugging until the flare has visibly calmed, then tapering to a maintenance schedule of two to three nights per week on the affected areas.

For age-related severe xerosis — regular use may be appropriate, but under dermatologist guidance rather than as a self-directed strategy.

Slugging every night indefinitely on already-healthy skin does not provide additional benefit — the marginal returns flatten once TEWL is back inside the normal range [2]. And extended nightly slugging beyond about two weeks without reassessment increases the risk of the sealed-environment problems above, even in people who tolerated the first week fine.


The reframe

Slugging works. When the underlying problem is mechanical barrier disruption with low inflammation, petrolatum is one of the most evidence-supported single interventions in over-the-counter skincare.

But it is not a universal nightly ritual. It is a targeted tool that closes the largest possible leak in the barrier for a defined period of time, giving the underlying skin the stable conditions it needs to repair. On the right skin, at the right time, in the right sequence, it is the difference between a barrier that spends weeks fighting environmental water loss and a barrier that can finally rebuild.

On the wrong skin — inflamed, sebum-heavy, or Malassezia-prone — the same seal creates the wrong environment underneath and produces new problems. The question is not whether slugging works. The question is whether the specific skin condition you are treating is one that benefits from full occlusion, and for how long.

Match the tool to the problem, keep the course short and defined, and slugging is a genuinely useful intervention for barrier recovery. Use it without triage, and it becomes another product that solves one problem while creating another.
